According to investigators, mafia infiltration has altered the functioning of the Reggio Calabria health authority, which was dissolved due to the mafia two years ago. The investigations, concluded in 2018, documented the organizational structures of the Piromalli gang with some doctors, including the deceased Giuseppantonio and Francesco Michele Tripodi who assumed important positions covering various roles in the health authorities of Reggio Calabria, Gioia Tauro, Palmi and Tropea. The suspects, through their prominent positions and the intimidating violence of the associates of the Piromalli gang, have «compromised the management system of the health districts of the ASP of Reggio Calabria, acquiring a dominant position in this context» the investigators write. Among those under investigation is also Fabiano Tripodi, son of one of the managers and owner of various healthcare companies.

According to investigators, mafia infiltration would have altered the procedures for appointing the director of the Tyrrhenian District of the ASP of Reggio Calabria and, through some companies, would have monopolized the economic chain of distribution of medical products to public hospital facilities of the ASP itself. In some cases, to facilitate companies attributable to the Piromalli, "the use of procedures for direct assignment of orders was found - in particular for the hospitals of Locri, Gioia Tauro, Polistena and Melito Porto Salvo - favored by the mediation of medical personnel rewarded with various benefits and undue commissions (variable between 2.5 and 5 percent of the nominal value of the orders)".

The investigations also revealed criminal collaboration between the Piromalli and the Molè, who appear together in some corporate structures. Furthermore, during the operation a preventive seizure of assets was carried out against some companies for a total value of eight million euros.
